How to make a referral

HOW TO REFER

WHO CAN REFER

  • Health professionals.
  • School SENCOs.

Please note we do not accept self-referrals from children and young people or parents and carers or referrals directly from the voluntary sector.

INFORMATION TO INCLUDE WHEN MAKING A REFERRAL?

  • Child or young persons and family details, including a telephone contact.
  • Referrer contact details.
  • Summary of past medical history and current problems.
  • Reason for a physiotherapy referral.
  • School or nursery attended.
  • Has the school pack been completed? – for children with co-ordination difficulties.
  • Safeguarding concerns.
  • Does the child have an EHCP?

It is always expected that consent has been obtained from someone with legal parental responsibility for the child before a referral is made.  Young people aged 16 and above with capacity can consent to a referral. Some young people under the age of 16 who have the capacity to consent to a referral can also do so, although it is essential that a person with legal parental responsibility accompanies the young person at least to the first assessment appointment so that a detailed childhood history can be taken.
