The Supporting Independence Programme
• Have you noticed any recent changes in your health? • Do you have difficulties walking or need help moving about? • Are everyday tasks such as shopping becoming more difficult? • Do you need someone to help you on a regular basis? • When you need help, do you sometimes feel lonely or isolated?
The Supporting Independence Programme provides up to 12 weeks of one to one support for those of us who may have slowed a little, are feeling less confident getting around or who need someone to help on a regular basis.
This could be supporting you to increase your levels of physical activity, helping you to access medication reviews or providing emotional or nutritional support and advice. Support can be face to face or over the phone depending on your preference.

If you're returning from hospital, we make sure you have everything you need for your first 24 to 72 hours back at home. We may then provide up to 12 weeks of support depending on the level of help you need.
What help you will get: We'll assess your needs and help you decide what goals you want to achieve with our help. We can offer practical and emotional support including: • Transport home from hospital. • Door-to-door transport for essential health-care journeys. • Help with everyday tasks (for example, picking up prescriptions and shopping). • Companionship. • Rebuilding confidence. • Help arranging for bills to be paid. • Short-term use of a wheelchair and toilet aids.
Our volunteers receive full training to support you in meeting your goals.

Meals with care Meals with care is a service run by Age UK. A volunteer will visit an older person at home to prepare a healthy light meal or snack and stay for a chat.
Help with digital skills Not everyone is familiar with the internet or has access to a computer to enable them to actually do their shopping or banking online. Or they may not have the confidence to do this for themselves.
There are services around such as Barclays Digital Eagles which, in conjunction with Age UK, offers support in a variety of ways to their more mature customers who may not be overly IT confident and would like some extra help.
